PCI is excited to partner with KidneyCare Oklahoma in providing their patients with a comprehensive Chronic Care Management program. This program will allows KidneyCare OK patients to interact with their providers on a more frequent base and help fill gaps in care. Nephrology patients many times have complex chronic coniditions and assuring that they are recieving continuity of care is vital to their health.
