CCM services are specifically for Medicare patients who have two or more serious conditions that are expected to last at least a year. As a large independent specialty practice, Advanced Orthopedics of Oklahoma has many patients with a primary diagnosis related to urology as well as additional chronic conditions like arthritis, asthma, diabetes, hypertension and more.
