Pro Care Innovations is pleased to announce their partnership with Urology Austin. Urology Austin is a group of over 40 urologists with 19 clinic locations in the Austin metro area. Pro Care Innovations provides them with Chronic Care Management (CCM) services for their patients.
CCM services are specifically for Medicare patients who have two or more serious conditions that are expected to last at least a year. As a large independent specialty practice, Urology Austin has many patients with a primary diagnosis related to urology as well as additional chronic conditions like arthritis, asthma, diabetes, hypertension and more.
