Pro Care Innovations is pleased to announce their partnership with Urologic Specialists. Urologic Specialists is a group of 21 urologists with clinic locations in Oklahoma, Arkansas and Missouri. Pro Care Innovations provides them with Chronic Care Management (CCM) services for their patients.
CCM services are specifically for Medicare patients who have two or more serious conditions that are expected to last at least a year. As a large independent specialty practice, Urologic Specialists has many patients with a primary diagnosis related to urology as well as additional chronic conditions like arthritis, asthma, diabetes, hypertension and more.
“We use Pro Care Innovations as our integrated source to provide chronic care management coordination,” said Matt Keep, CEO of Urologic Specialists. “They have dedicated care coordinators specifically assigned to our practice that contact our patients monthly to proactively identify any health questions, concerns or issues our patients might have. They then provide immediate feedback to our physicians and nursing staff on any issues and serve as a resource to ensure that our patients have good interaction with our practice at all times rather than just during their appointments.”
